Health department suspends provider credentials in Lewis, Thurston counties

The Washington state Department of Health (DOH) suspended the credentials of four healthcare providers in April and May, according to a recent news release from the agency.

Lewis County

In April, the DOH indefinitely suspended James R. Smiley’s agency-affiliated counselor credential by default order, after Smiley failed to respond to a statement of charges.

The charges, issued in September 2025, followed Child Protective Services’ finding that Smiley was “responsible for negligent treatment or maltreatment of a minor child,” the DOH said.

That finding — first determined in 2023 — became final in January 2024, according to the release.

In May, the DOH indefinitely suspended Christopher Bernard Otieno’s registered nursing assistant credential by default order, after he failed to respond to an April statement of charges.



According to the release, Otieno “reported to work appearing intoxicated” in July 2024. He later admitted to consuming alcohol, the DOH said.

Thurston County

In April, the DOH indefinitely suspended Alyssa Marie Martinez’s substance use disorder professional trainee credential.

According to the release, Martinez “failed to provide documentation of required supervision” while working at a Lakewood clinic between April and November 2024.

In May, the DOH indefinitely suspended Ryan Shaw’s substance use disorder professional trainee credential by default order, after Shaw failed to respond to a February statement of charges.

According to the release, in June 2025, "Shaw reported a relapse while working as a behavioral health technician and drug counselor at a treatment center.”
